Wisdom tooth: extract or watch

Not every third molar needs a “preventive” extraction. The decision follows a radiograph, not a group-chat scare story.

Wisdom teeth erupt at an angle, press on the seven, inflame the gum flap — or sit quietly for years. A simple extraction on our list is about UAH 1,650; a wisdom tooth about UAH 2,850. A difficult impacted eight, sedation, nearby implants — different fees and a different plan.

When we watch

The eight is in bone, no cyst on the film, it is not crowding the neighbour, you do not feel it. Then follow-up, not an operation “just in case”. A repeat radiograph on the dentist’s schedule.

When we extract

Repeated pericoronitis (the flap flares again), decay on an eight you cannot clean, pressure on the seven, a cyst, space for orthodontics if the orthodontist has justified it. “Because everyone has them out” is not an indication.

What the visit looks like

Radiograph, anaesthesia, extraction, home instructions. Swelling and limited opening for a few days is expected. Sedation — if there is fear, a difficult access, or several teeth at once, not “because Instagram”.

After extraction

Do not rinse hard the first day, do not poke the clot with your tongue, do not heat the cheek. Bleeding that will not stop, fever, a sharp smell and rising pain after a few days — call, do not forum-shop. A gap after an eight does not always need an implant: it is the end of the arch.

Wisdom teeth: frequent questions

Do all four have to come out at once?

Not necessarily. Sometimes two sides in separate visits is wiser. “All four under sedation” makes sense when all four actually need to go and you choose one anaesthetic day on purpose.

Can I keep it if it is already coming through?

Yes, if you can really clean the flap and there are no repeat flare-ups. An antibiotic every month is not watchful waiting — it is postponing the extraction.

How long off work?

Desk work is often the next day. Physical work — better a few days. The dentist will be more precise after seeing how deep the eight sat.

Should I put an implant where the eight was?

Usually no. The seven covers chewing at the back. We discuss implants when teeth are missing further forward, not “so there are eight teeth like at 18”.
